Incident was reported 3x the following have been deleted: 173528 & 173564 At approximately [REDACTED] employee [REDACTED] walked into the office and informed me that employee [REDACTED] was hurt out on the work room floor. He also stated that he could not move and the cause of the injury was caused by the chair he was sitting on had collapsed. [REDACTED] was on the floor on APBS console #1. I asked him if he was alright and nodded his head side to side and said no. I instructed him to try and not move as I called paramedics. Paramedics arrived at approximately [REDACTED] where he was evaluated and taken to the hospital on a stretcher.
