What happened
On [REDACTED] at was dispatching two full boxes of mail operating a double jack. Employee [REDACTED] entered the west dock and parked the mail in appropriate staging lane. Employee was pulling away from the staging area and the jack began to shake really bad throwing her head into the handle. The faulty jack lost power ejecting my body off the jack.
