On [REDACTED] between [REDACTED] and about [REDACTED] Carrier says they think they were cutting across a lawn on [REDACTED] OR. The exact address is unknown by carrier and Managment could not find it later in their search for it. She stepped where it looked like solid ground and it was an overgrown pothole. Her right ankle and lower back were twisted she claims she could not bend over but could walk so she continued her route and didn 't report it until the end of the day. Management took DMS breadcrumbs to try to locate the location of accident and could not determine exactly where it was. No onsite investigation was conducted.
