Employee stated that her right eye was infected from the dust caused by the construction to the roof. On [REDACTED] the employee declined to write a statement and declined medical services. Employee wanted to go to own doctor the following day stating that she has bad allergies and the infection irritation might be from allergies or might be from flowers she bought the other day. Later in the day on [REDACTED] she did write a statement saying that there was an injury to their eye by the dust caused from the roof repair and said she wanted to go to her own doctor. A picture was taken of her right eye by management.
