What happened
Forklift operator stated that his blades were raised up blocking his viewpoint and while the carman was guiding the truck driver into the correct position for his store to be taken off. The forklift operator 's foot slipped and pushed on the gas pedal. This caused his lift to pin the carman in between the truck and the forklift (between the blades) and started to crush the carman.
