Unloading cylinder truck of empty Nitrogen cylinders and placing them on a metal pallet; so cylinders could be refilled.

What happened

The employee was transferring empty Nitrogen cylinders (K size bottles) from a cylinder transport truck to a metal pallet for refill. There was a three-inch rise between the base of the metal pallet and the floor which required the cylinder to be leaned backward to get it onto the pallet. As the cylinders were being transferred to the pallet the following happened: • As one of the cylinders neared the metal pallet the employee leaned the cylinder backwards but lost control of it and it started to fall. • The employee grabbed the cylinder by its cap to keep the cylinder from falling. • The cap appeared not to be secure on the cylinder; so the cap came off of the cylinder and struck employee above his left eyebrow. • Employee fell back and called to other employee for help • Other employee was near injured employee applied first aid and called Supervisor who immediately contacted [REDACTED] • Injured employee lost consciousness for a short period of time. • Injured employee
