Dept: Training on stand-up Reach in Service Area [REDACTED]: DOI: 2 9 23: TOI: 1030: Started Work: 1000: Shift: 1st: On Job: 1 2 hr: Witness: [REDACTED] Left thigh Lower Leg: Per EE statement: EE was training on Reach truck driving along the long aisle. As she was driving there was another Reach coming down aisle [REDACTED] carrying a pallet. The other driver did not notice EE and hit her Reach on the right-side causing EE to by thrown to her left side in her Reach.. Left leg was on the [REDACTED] in an awkward position and it twisted a little. Also left thigh hit the left side of the truck. Initially did not notice anything but [REDACTED] started to get pain in left calf and left thigh area.
