At the time of the incident reporting we have been doing conservative treatment with the TM. On the morning of [REDACTED] she came into work early and was placed in same area where she hurt her shoulder on [REDACTED] an reaggravated it. After coordinating with her supervisor [REDACTED] she left and went to [REDACTED] [REDACTED] to get checked out. [REDACTED] noted on her Occ [REDACTED] Report “[REDACTED] RTW with No restrictions when cleared by Occ [REDACTED]” and was provided pain and muscle relaxing medications. She was advised by occ health to rest should for a couple days and to call Occ heath in [REDACTED] [REDACTED] to make a follow up Appt. HR drove her home as she was feeling some of the effects of the medication already.
