Before the incident
He was walking and observing the quench tank when he noticed that the pipe was not flowing with carbon and hot water.
What happened
Once he knew that the pipe was clogged with carbon he proceeded to the bottom of the tank to open a valve when the carbon blocking the pipe fell to the quenching tank splashing the back of his head burning him.
