What happened
While [REDACTED] was working an oil sample caught from an oil tank the sample glass slipped out of his hand. He was wearing latex gloves at the time. The [REDACTED] attempted to catch the sample bottle but in turn shot the glass bottle towards his face hitting himself in the mouth. The bottle contacted his front tooth which resulted in chipping it.
