What happened

On [REDACTED] while working on DB19 at approximately [REDACTED] employee stated that he felt his left shoulder lock up while performing his duties after turning over to second pass. He went to supervisor and told him that he could not continue to work because he was in a lot of pain and could not move his left arm. He asked for a 3971 to go home but did not say to the supervisor that he hurt himself while performiing his duties. He reported to work on [REDACTED] to request a CA-16 at which time he told Supervisor what had taken place and informed her that he had a Dr. appt. the following day.
