What happened
On [REDACTED] at [REDACTED] [REDACTED] contacted Supervisor to alert him that she was feeling lightheaded and dizzy. Supervisor instructed her to stay at her location at [REDACTED] and Supervisors to her location and drove her to a Concerta Health clinic at [REDACTED] in the [REDACTED] staff car. The clinic was closed and when [REDACTED] exited the vehicle she collapsed into [REDACTED] 's arms. [REDACTED] then called [REDACTED]. An Ambulance took [REDACTED] to St Joseph Hospital where she was discharged at [REDACTED] with a diagnosis of dehydration and an ear infection.
