On [REDACTED] at approximately [REDACTED] employee reported that she was injured while on duty [REDACTED] prior she stated she was handling a heavy package on the orange hamper when she hurt her back that particular day she didn 't report an injury or accident while on duty. she went to the emergency department and was off work some couple days until we called her up to see what was going on in with the her she then stated she was injured while at work.
