While exiting his vehicle with a package in his hand he slips on the footstep -well hitting his right knee on metal door frames this incident occurred between [REDACTED] on [REDACTED] I ask him if he was requesting medical treatment. He refused medical attention that day. On [REDACTED] at [REDACTED] Mr. inform Ms. A Supervisor that he went to Urgent Care on [REDACTED] and seek medical attention and brought paperwork. Carrier is filing a claim.
