Employee filed a CA-1 in Ecomp stating the following: Forklift driver ran over a pallet jack causing it to raise on one end when she came off the pallet jack the jack then leveled itself back out while coming to a rest on my left foot. My left foot was crushed by a pallet jack causing me to have X-rays done to which the doctor says is a very serious contusion and to stay off my feet.
