What happened
On [REDACTED] at approximately [REDACTED] I [REDACTED] received a call from [REDACTED] through the work room floor. [REDACTED] told me that he had fallen and that his right elbow was hurting as a result of the fall. I told [REDACTED] that I was on my way over to see what happened since I couldn’t really make out what he was saying on the phone. As I got to the AFSM100 [REDACTED] approached me and said that at the [REDACTED] of the [REDACTED] run they were loading the mail for zone79936 onto the consoles. He walked by an empty Web GP not realizing that his left foot had become entangled with the GP webbing. As he fell he struck his right wrist and elbow. [REDACTED] said that no one was around who witnessed him falling but that PSE [REDACTED] and [REDACTED] had both seen him on the floor.
