Mr. [REDACTED] summitted a ps form 13 with the following statement: On [REDACTED] while performing duties of moving empty and full equipment APCs OTRs wire cages and pallets of Amazon packages I injured my back believing I just was hurting because of the repetition. But when I went home it was still hurting. I woke up the [REDACTED] on [REDACTED] to report to work in excruciating pain. I could barely move without the pain shooting down my back and my back was also stiff. I could not make it to work. I woke up on [REDACTED] feeling the same way. It is very uncomfortable to sit up or move around. I am requesting to see a doctor as this was an injury that occurred on the job.
