Before the incident
[REDACTED] was on sight at City [REDACTED] in [REDACTED] where we have a storage unit to receive a delivery of a large crate and pallet which held at an event. From eyewitnesses what occurred from there was that [REDACTED] was helping the driver as a spotter to back the truck into the loading area of the warehouse.
What happened
he driver was attempting to position the crate unto the liftgate but because he was alone [REDACTED] was attempting to assist from the ground. At some point the crate rolled off the liftgate falling onto [REDACTED] and pinning him under.
