Employee was off loading barrels from the flat bed truck with the use of the fork lift. He drove off the coencrete pad and got stuck in a pot hole.

Employee used the flat bed to try and pull the forklift out with the use of chain. The chain was wrapped around the rear of the flat bed and chain to the mast on the forklift. The yardman was given the ok to pull by the employee which resulted in the fork lift tipping over.
