Before the incident
Two Maint. Technicians were preparing to replace a section of a steamm pipe damaged from a forklift operator.
What happened
As the close off vaslve was being loosened for removal water along with high pressure steam started flowing from that section of the pipe. The hot steam burst through the opening burning the employee 's left arm. No LOTO procedures were followed.
