Before the incident
Worker was loading totes on a rack for a putaway associate.
What happened
Associate 's ID badge was caught in the tote and when they placed the tote on the rack the badge was pulled back where it struck their eye. The worker alerted the Lead and an Incident Form was filled out. Worker went to Concentra to get checked out as his eye was red and puffy. Concentra followed up with EHS Manager and let him know they sent him to the ER to be checked out further.
