TM was disposing of cardboard in the dumpster prior to returning to the production line

TM 's foot was caught in the rope used to close the roll up door that he had just closed. As TM walked away from door he tripped and fell to the ground.

TM Contacted the ground resulting in a 3 4 of an inch laceration above the right eye. Visible swelling on his eyebrow. TM also stated his right side ribs were sore to the touch.

The Production floor of the warehouse (Concrete) The Rope from the Roll up Door was the root cause of the trip.
