What happened
Employee was working as a hatch lead [REDACTED] in Hatch 5 aboard a vessel discharging bagged cement. He was walking across a row of bags and was getting out of the way of a spreader bar that was lifting a load of bags out of the hold. He stepped on an empty bag that appeared to be full of product from his point of view. Once he began stepping his foot fell through a hole between bags and the injury occurred.
