EMPLOYEE HAD ONE HEADPHONE IN HIS EAR AND MOST OF THE CORD WAS TUCKED DOWN HIS SHIRT. THE OTHER HEADPHONE WAS HANGING DOWN OUTSIDE THE SHIRT. WHEN STACKING THE BOX ON THE LINE AND THE LOOSE HEADPHONE GOT CAUGHT BETWEEN THE BOXES AND FLUNG BACK HITTING HIM IN THE LEFT EYE.
