On [REDACTED] At Approximately [REDACTED] Ds [REDACTED] Injured Her Right Big Toe Nail By Dropping The Lip Of The Lift Gate On Her Foot At Stater [REDACTED] 84 Ds [REDACTED] Stated That When She Arrived To The Account There Was A Truck Already On The Dock While She Waited She Decided To Load An Empty Cart On The Truck Using The Lift Gate When She Finished Loading The Cart She Went To Close The Lift Gate And It Landed On Her Foot More Specifically Her Big Toe She Immediately Felt Pain And Removed The Lift Gate From Her Foot Ds [REDACTED] Took Off Her Shoe To Notice Her Toe Was Bleeding After Connecting With Ds Lead [REDACTED] She Was Connected With Dsl [REDACTED] At Which She Asked To Go To The Clinic To Have Her Toe Assessed Ds [REDACTED] Was Wearing All Proper Ppe Including Slip Resistant Shoes And Gloves
