Employee was helping a coworker stack material when his lower back began hurting. He voiced he walked over to the forklift to stretch out his back. He states he began feeling dizzy then passed out. He voiced his coworker reports he passed out hitting his head on a part of the forklift.

He states he did suffer a laceration to the back of the head and voiced he was told he was only unconscious for 15-20sed. Mr. [REDACTED] voiced the ambulance was called and he was taken to Hanover Emergency Center [REDACTED] where he is currently being treated. He states he is about to be seen by the doctor now and is waiting on stitches. When asked if he has had a CT scan or any other studies competed he states he is currently waiting to be taken back for a CT scan
