What happened
While running a C S CIP on [REDACTED] I needed assistance putting on the last 2 inches of the manway door gasket. I called over my supervisor [REDACTED] to help me. In order to be safe I felt it best that I stood in the ground and support the ladder. [REDACTED] first took off the handle to the manway and while helping me put on the gasket the crossbar of the door fell on top of my head.
