Feeding metal through the Roll Former
What happened
Temporary EE was trying to feed metal through the Roll Former when she accidentally hit the toggle switch causing the rollers to pull her left hand into the machine. The EE immediately puller her hand out amputating the tip of the left thumb and the nailbed of her first finger on her left hand. The employee was only supposed to be working on labels. She was not trained or authorized to work on the machine. When the operator went to the restroom the temporary employee took it upon herself to start playing with the controls
