Before the incident
[REDACTED] was returning from break. She explained that she had forgot her phone and had to go back to get it and realized she was running late so she was walking very fast. She slipped as she was coming down the hallway. There was no wood water or other items on the floor or in the area she could have slipped on. Her shoes are like new and in good condition. She was sat in the office for 40 minutes with ice and elevation and the ankle was taped up with an ace bandage. She spent the remainder of the shift watching safety videos will contact Work Partners if no further improvement within two hours.
