Before the incident
The TM was cleaning [REDACTED] filler bowl and when he noticed there was water coming out of the product pipe he stopped what he was doing to find out why water was coming out and that is when he slipped and fell down the stairs and hit his back and head. TM thinks that he had slipped on apron cause it was too long. Was advised he needs to cut apron shorter. He was also advised to meet with [REDACTED] [REDACTED] before starting his next shift.
What happened
Employee was spraying out filler bowl on [REDACTED] when he noticed additional water filling bowl. As he went to investigate cause of water he turned began walking down stair and slipped on his work apron causing him to fall on to his back.
