What happened
Home made hook used to hold the poly tote liner bag of empty tubs became stuck with the bag. two employees on foot and one employee on a fork truck were used to pull the bag of tubs free from the hook. the hook was loosely on the guard rail and once the weight of the bag was removed the hook sprang forward and came in contact with the chest of the employee.
