A collet was stuck in the Jacobson mill. TM attempted looking for the removal tool and was not able to locate it. In response to this he used a steel rod to tap the chuck in a downward motion to help relieve the pressure holding the collet in. The chuck separated from the collet and fell approximately six inches and landed on the TM 's right pinky finger. The TM stated that his right pinky finger swelled a little bit and he finished his shift and left. He was at home when the pain became significant. He decided to seek medical attention on his own at [REDACTED].

The EE saw a chuck stuck in a drill press of the Mill. The EE tried to find the collet remover for the chuck and could not locate it so a steel rod was used to tap the drill press to remove the chuck. The chuck fell and pinched the employee 's finger.
