What happened
Employee went into the freezer located inside the walk-in refrigerator to get the frozen chicken tenders. As the employee was coming out of the freezer to step walk into the refrigerator section the employee slipped from the water that had accrued overtime that was outside the freezer door. When she slipped and fell backwards she landed on her tailbone and back.
