Guiding a large metal roll into a lathe

A large (about 5-feet long) metal roll was being moved into a lathe by a fork-lift. The employee was guiding the end of the roll into the lathe when the roll shifted and slid out of its protective covering. The employee and the fork-lift driver had not used the normal procedure of using a choker hitch around both ends of the roll but just had a strap around the roll and its protective covering. When the roll slid out it slid to the floor and onto the employee 's right foot. The employee said he was wearing his steel-toed boots and the roll hit him on the bridge of the foot. The supervisor was not notified and the employee worked the rest of the day until about an hour before the end of shift. He then told the supervisor what happened and his foot was pretty sore and that he was going to go home early to rest. He was then advised to go to the emergency room. Employee was told to take one week off from work then he should be fine to return. I called the employee the following [REDACTED]
