On [REDACTED] [REDACTED] and I were cleaning up the OR after a [REDACTED] section that had been performed by Dr G [REDACTED] cleaning up appropriately I presumed to walk down the hallway to take the dirty linen cart to the decontamination As I walked down the hall not knowing that [REDACTED] had copped through the hallway I slipped and fell landing with my left leg twisted behind and under my buttocks with my left ankle hips and right wrist taking most of the impact [REDACTED] was behind me at the time and witnessed the incident
