Pt recieved seated in recliner with RN present at bedside BP low 9850 with MAP 91 upon PT arrival RN holding meds 22 hypotension PT declined ambulation opting for STS prior to BP reassessment to monitor BP response to exercise Patient denied lightheadedness or dizziness We were standing in front of the recliner with bariRW positioned anteriorly for safety On the 5th stand pt fell forward pushing the RW out of the way and fell to her kneeshands before sliding into prone PT present and assisted at pt shoulders to control decent and prevent her head from hitting the floor No LOC I did not witness a knee buckle LUMBAR SPINE PC
