Patient woke up and went to the nurition room Patient followed direction to back up from the door to allow me to go in and get him a snack Patient then charged me and the door door was closed and he was unable to get in Patient then grabbed my hands and pulled me to the door I was able to get him to release I returned to my spot and patient charged me swinging striking me in the face 2x and grabbed me by my hair 3 Staff members assisted in getting him to release me
