About to come off added evening shift. Patient found on bathroom floor.
What happened
Patient required all 9 staff to return him to bed. When getting patient into bed she was caught in between the patient and bed. The bed 's position did not remain locked when it was adjusted to low level. This caused her to be pulled forward and twisted her up onto the mattress where her left knee was then pinned and popped between the bed and the patient.
