Employee supervises the Flashings department and had found a hammer head without a handle. Employee decides to go to Packaging department and cut a piece of wood to make a new handle for the hammer head.

Employee was using a table saw to try and cut a hammer handle for a hammer head. Employee had cut the wood into a very narrow strip and ran the tip of his left thumb through the saw blade removing the tip of his thumb.

Amputation to the tip of left thumb

Employee found a hammer head with no handle and decided to fabricate a handle for the hammer head on the table saw. While trying to cut such a small piece of wood the employee pushed his hand into the saw blade while it was operating.
