What happened

Distributor operator stopped the leveler to remove small rock that was stuck in the Leveler that was on Section C on the 5th bed. Employee tried to engage the E-Stop. Because of the rock the employee thought the machine was off. Employee stuck his arm in the leveler and removed the rock. When the rock was removed the leveler started up and pulled the employee 's arm in and had his arm caught. Other employees cut the power from the outside. Emergency services were called. Employee 's arm was removed from the leveler by emergency services. After looking at machine with repair staff and the supervisor the E-stop was bent so it couldn 't be engaged. Also the valve shut off lever was very loose and can be spun around and not stop at the shut off position.
