Before the incident
[REDACTED] was getting pattern out of heater with fork truck. While still on the fork truck he went to close the door. He accidently let his foot off the brake inching the lift forward causing his left hand to get pinched between the door and the truck.
What happened
At the time of his incident he was attempting to remove a pattern from one of the heater boxes. He was still on the fork truck when he went to close the door. His foot was engaging the brake and it slipped off while reaching out to shut the door. This caused his hand to get pinched between the door and the truck.
