Before the incident

[REDACTED] was using a grinder with cutoff wheel to clear jammed up sprue gating in the the didion crusher. Finished work at the didion and took the next call to the packout cell. Repaired then went to break removed his PPE and felt something fall in his right eye.

What happened

[REDACTED] felt something in his left eye at [REDACTED] when he was doing his inspections. He is not sure how anything got in his eye. He did use air to clean off parts for inspections several times throughout the night. He did come in early at [REDACTED]. We are not sure where the object came from or when just that he first noticed it when taking parts to quality to be checked. He tried washing his eye out but could not get the object out. He mentioned it to his AL who directed him to his GL. We went to the first aid room and washed his eye out three times but could not get the object to move.
