What happened
The [REDACTED] operator ([REDACTED]) had just pulled a full scrap pallet from his line and was getting ready to load an empty pallet for the good product. As he was backing the electric forklift to line up with the table on [REDACTED] [REDACTED] maneuvered an empty pallet jack behind the forklift. The left rear wheel well caught [REDACTED]’s left foot. The rotation of the wheel and rearward momentum pulled his foot up and into the wheel well. [REDACTED] fell forward to the floor and pulled his leg free while his shoe remained stuck in the wheel well.
