He had a little giant ladder folded out leaning up on a wall. He was going to pull a screw with his impact and applied force. When he pushed forward the bottom of the ladder slid backwards and he came down with it. His left leg fell in-between the ladder and the ground so when he hit the floor the leg got caught and pinched
