What happened
Staff went to client 's bedroom to check on them. Bedroom door was closed so staff knocked several times & called client by name. Client didn 't respond - she knocked one more time & told client she was coming in the room & as she opened the door the client slammed the door to close it & it struck the staff in the head neck area.
