Before the incident
Removing the wire rack that is near door 6 from a ladder which was secured with sandbags.
What happened
Employee was standing on the 3rd level of the wire rack to remove another wire rack planning to place it on a forklift that was raised with a pallet nearby to avoid walking up and down the ladder. After removing three of the wire racks the front horizontal orange beam came off from the green vertical rack causing the employee to fall he landed on his feet which created the injury.
