What happened

On [REDACTED] [REDACTED] contacted his supervisor via phone call at the end of his shift to report discomfort and pain he was experiencing to the left side of his groin. Over the course of the conversation [REDACTED] could not recall nor provide details as to when where and what time it occurred. He verbally stated that he will make an appointment to see his PMD. On [REDACTED] [REDACTED] contacts his supervisor in the evening stating it was a work related injury. Communication was then made to [REDACTED]’s Manager and Safety Manager. Safety met with EE [REDACTED] on [REDACTED] and EE mentioned he had hurt himself Throwing Cases but was not able to recall what day which account and specific time of occurrence. EE was not able to recall exactly what type of bodily movements he was performing and that the pain was gradually increasing. EE then mentioned that he had felt the pain on [REDACTED] thinking it was a pulled muscle thinking I will “go away” on it’s own. He then disclosed
