Before the incident
Checking wheels behind the automated line (#11) because buff was light. Operators forgot Setup was behind the line and started it. Employee did not follow proper Lock Out Tag Out procedures.
What happened
Employee 's coat sleeve was caught in the mechanics of the auto line. He was able to get to the emergency shut off but not before his right shoulder and lower back was strained.

Lower back and right shoulder strain. A week later the employee was reporting a bad headache and wanted to be seen by a doctor to make sure it was not somehow related to the incident.
