[REDACTED] was standing outside the building at the entrance to [REDACTED] on the South end of the main shop observing and giving instructions to [REDACTED] who was driving a forklift.

[REDACTED] was driving a forklift into the building with two 30 ' long I-beams chained onto the forks. [REDACTED] observed that the chain was too close to the end of the forks and was attempting to warn [REDACTED].

When [REDACTED] drover the metal transition piece at the bay door the beams slid off the end of the forks and landed on [REDACTED] right leg crushing it and pinning him to the ground.
