Using a torque wrench to tighten bolts.

The employee was in a boom lift and approximately 10 feet in the air torquing bolts. He was reaching outside of the lift to grab the wrench with both hands and pull it towards his body. In a single motion he would pull the wrench down towards his chest until the wrench signaled the proper torque. The motion he was making was similar to a pull up. While cycling through he bolts he pulled down on the wrench and his right shoulder dislocated from its socket.
